1. What might the surface ECG tell us?

The surface rhythm is irregularly irregular with absent organized P waves in typical AF.

2. What might we see intracardially?

Intracardiac recordings show rapid, variable atrial activity; modern procedures integrate signals with 3D anatomy.

Technologist mindset

Describe the sequence before naming the mechanism. Ask what is earliest, what follows, whether atrial and ventricular activation are linked, and whether the pattern changes with pacing.

3. Pacing clues

The procedural strategy is not based on a single diagnostic pacing maneuver; pacing may be used for other procedural assessments.

4. Mapping concept

Contemporary AF ablation centers on pulmonary vein isolation, with additional strategies individualized to patient and procedure.

5. Ablation concept

The core ablation concept is durable electrical isolation of the pulmonary veins; technologies include thermal energy and pulsed field ablation.
